1. I hereby attest that I am eighteen years of age (18) and that all of the information provided is accurate and true to the best of my knowledge.
2. I hereby authorize Split Skull Media, to publish, edit, change or copy photographs herein, taken of my personal property and my name, described as artwork for use in Split Skull Media’s print, online and promotional brochures, displays, websites, social media, products, activities, video-based materials worldwide, and materials, in perpetuity, as well as any other company publications for any legal usage.
3. I hereby attest that the photographs taken are genuine images of original artwork that is owned, created and/or photographed by me.
4. I hereby release and hold harmless, Split Skull Media, from any reasonable expectation of privacy or confidentiality associated with the images specified above.
5. I further acknowledge that my participation is voluntary and that I will not receive financial compensation of any type associated with the taking or publication of these photographs or participation in company marketing materials or other company publications, with no exception. I acknowledge and agree that publication of said photos confers no rights of ownership or royalties whatsoever.
6. I hereby release Split Skull Media, its contractors, its employees, and any third parties involved in the creation or publication of marketing materials, from liability for any claims by me or any third party in connection with my participation.
7. I release all rights to any, but not limited to, claims, rights, demands, and/or any causes of action by me or my representatives, heirs or anyone else.

I have read, fully understand, and fully agree to comply with the statements and requirements listed herein. All information provided is true and accurate to the best of my knowledge and belief. I authorize Split Skull Media to use my photo(s) as outlined above. Any attempt to alter, deceive or falsify any information will result in legal action.
